Rental Application (Click Here) - There is no fee to apply for an Oak Haven home. We will use the application information to contact you and to pre-screen applicants before the required credit & background check. If, based on the information you supply in the application, it seems that you are a good fit for one of our homes, we will contact you about filling out a Request for Credit & Background Check form. We try not to have people pay for the Credit & Background Check if we are likely to offer the property to someone else. Employment & Income Verification (Click Here) - Please give this form to your employer for them to fill out and send back in to us to verify your employment and income. It requires your signature before your employer will release information to us. They can fax it in or send it in via e-mail. Previous Landlord Questionnaire (Click Here) - We must have this form filled out and turned in to us, or we can call your previous landlord and collect this information over the phone. Request for Credit & Background Check (Click Here) - We are required to have a signature on this form, so it can not be filled out online. We do a credit and background check on all members of the household who will be responsible for the payment of the rent ($30 plus $1.99 credit card payment explained below). We also run a background check on all other adult members of the household ($20). Please print out this form and either drop it off or send it in with the fee (payable to Oak Haven Investments, LLC). The credit check also requires an online payment of $1.99 (payable with a credit card or Visa or Mastercard debit card) to verify credit information. This additional fee is for your own security and will be requested by the reporting agency to verify that you are authorizing us to access your information. Rental Lease (Click Here) - Please take the time to read our lease fully before signing. If you have any questions, it is best to ask before signing, rather than risk a misunderstanding later on. It is a legally binding contract so do not hesitate to seek legal advice if you feel it is necessary. Pet Rider for Lease (Click Here) - Our standard lease does not allow pets. We will allow pets in some of our properties, subject to receiving permission for a specific pet in a specific unit. There is an additional monthly Pet Fee and an additional refundable Pet Security Deposit. This rider must be completed before any pet is allowed into a rental unit. Protect Your Family From Lead in your Home (Click Here) - "Many houses and apartments built before 1978 have paint that contains high levels of lead (called lead-based paint). Lead from paint, chips, and dust can pose serious health hazards if not taken care of properly." (US EPA) While none of the homes that we have for rent have documented evidence of lead-based paint, it is likely that the older homes may contain lead-based paint under the newer coats of paint. It is wise to review this brochure on how to minimize your risks of exposure. Landlord and Tenant Rights and Duties Under Ohio Law (Click Here) - "The Ohio Landlord-Tenant Act of 1974 sets out minimum rights and duties of both landlords and tenants.* This publication was prepared to raise public awareness of the rights and duties related to renting a home. It should not be substituted for the advice of an attorney. Differing facts and local custom can change the application of the law.
